Gravity cycles run 30 to 60 minutes at 15 PSI. Pre-vacuum cycles run 4 to 18 minutes at 30 PSI, but they require a Bowie-Dick test every morning to verify air removal. The critical advantage is cost per cycle – pennies compared to low-temperature methods. The critical limitation is material compatibility. Never put powered drills, cameras, or any instrument with sealed bearings or lenses in a steam sterilizer. SECTION 2: LOW-TEMPERATURE ALTERNATIVES – ETO AND HYDROGEN PEROXIDE

When your instruments are heat-sensitive, you have two main choices, and they are not interchangeable. Ethylene Oxide (ETO) gas is the old reliable. It penetrates lumens and complex geometries beautifully, making it ideal for long, narrow endoscopes and delicate electronics. For a busy ambulatory surgery center, plasma is often the right call. For a large hospital with complex robotic instruments, you will likely need both ETO and plasma.

Start with a reliable pre-vacuum steam sterilizer from a manufacturer with a local service technician – that is your backbone. Train your central sterile staff on load density and water quality every six months. Sterilization failures are almost never machine failures; they are human and process failures.
